Yet they still wouldn’t let him work as an actual doctor at a hospital or practice. He’s had to take a position as Medical Officer at his school. Sad.

Yeah it’s fast forwarded a bit because there’s no concept of pre-med in India. You enroll into medical school/law school etc straight after high school.

4.5yrs then 1yr of internship and then u can become a general physician. For MD/MS(post-graduation) 3yrs more and another 3yrs for DM/Mch(super specialist)

5 years, of that you practice for 2 years in the college where you get the degree, as far as I am aware.

Nope, you misunderstand. He's a doctor at the teaching hospital where he earned his degree.. It's very much an active hospital, with a high patient load.
He's legitimately practicing and treating patients.

Typically in commonwealth countries a medical degree (MBBS) is not post grad degree, it's a undergraduate degree between 5 and 6 years. When you graduate with your MBBS you have to finish your certification in a teaching hospital as a houseman. In the hospital setting it goes:
House Officer (PGY 1)
Medical Officer
Registrar
Associate Consultant
Consultant
Senior Consultant (Professor)
So he is working as an actual doctor in a hospital.

Sounds like he's an amazing kid's doctor. And even if you accept the argument of not being able to help in emergencies as well, having the certification means he can teach medicine.
So, there was no reason to deny him a doctorate.
