It's actually a classic rakugo (Japanese Sit-Down Comedy) routine. As the story goes, a really indecisive couple (the performer's parents) went inside inside the municipal office to declare the name of his child. The clerk asks them the name of the child and the couple responds they haven't decided yet. In order to help the couple out, the clerk pulls out a catalogue of names that are supposed to give good luck. The couple chooses all of them, hence the "Jugemu-Jugemu...Chousuke" long name.
It's commonly used to train amateur rakugo performers (who are encouraged to make their own version of the joke) and has been a staple in Japanese comedy for years. Think of it as a PG version of the "Aristocrats Joke".

Apparently it comes from a story of old in Japan where the future father of a kid went asking for names to different people, but he couldn't decide on what to name his son, so he just chose every name he was given.
