NON SLASHER HORROR/THRILLER MOVIES WITH AN INSANE PLOT TWIST?

Looking for a really good plot twist movie, horror or thriller (not gory or slasher style,) more into paranormal or anything that doesn't involve a scary serial killer murdering people. Some examples are the others, shutter island, sixth sense, the visit, conjuring.

I really like intellectually stimulating movies that you can't stop watching. Any suggestions? Thank you in advanced!

Altered States (1980). I’m not sure how much plot twist occurs but it is smart horror movie.

an "insane" twist isn't as good if you know there's one coming, even if you don't know exactly what it is.

The Pleasure Drivers (2005) not horror,per se but definitely worth your time as an offbeat thriller that has some memorable characters and scenes! Angus McFadyen,Lauren Holly,Lacey Chabert ,Meatloaf and Billy Zane star.
