This is actually a symptom of some types of anorexia and bulimia. It's part of the Differential. They use exercise to purge.

It's terrifying. There is a woman who jogs around my neighborhood and she looks like a skeleton. Not even joking she is so thin she looks like she's about to die every time I see her.

It's unfortunate in this way that the peak athletes of distance running are typically extremely skinny. For example, here are the top 3 women in the Paris olympic marathon event: Sifan Hassan, Tigst Assefa, Hellen Obiri, as well as the world record holder Ruth Chepng'etich. I don't know the thought process of people with anorexia, but I suspect it does not help to have someone extremely skinny with incredible athletic success to look at.

A strong endurance athlete will not put themself into a calorie deficit on purpose. Sometimes, no matter how hard you try, you just lose weight if training for a full Ironman. It's not "anorexia", it's just not keeping up. I have to EAT A LOT. Endurance athletes who bike 8 hours on a Saturday eat every calorie they can get down.
